SurveyScope ("SurveyScope," "we," "us," or "our") is a mobile application operated by NSPB Corporation. This Privacy Policy explains what information the SurveyScope app collects, how we use it, and the choices you have. By creating an account or using the app, you agree to the practices described here.

We have built SurveyScope to collect as little personal information as possible. We do not use advertising, we do not track your location, and we do not sell your information.

Account information. When you sign in, you authenticate through Microsoft Entra (our identity provider). We receive and store your name and email address so we can create and identify your account. You provide your first and last name when you set up your profile.

Subscription information. SurveyScope is a paid subscription. Payments are processed by Stripe, Inc. Stripe collects and handles your payment card details directly; we do not receive or store your full card number. We store your subscription status (for example, whether your subscription is active) so we can provide access to the app.

Support communications. If you email us for support, we receive the information you choose to include in your message.

To be clear about what SurveyScope does not do:

• We do not use analytics or crash-tracking tools.
• We do not collect your location.
• We do not use advertising or advertising identifiers.
• We do not track you across other apps or websites.
• We do not sell or rent your personal information to anyone.

SurveyScope displays public information about skilled nursing facilities, including survey citations, surveyor narratives, star ratings, and related records. This information is sourced from public government data (such as the Centers for Medicare & Medicaid Services) and public state records. It is public information about facilities and is not personal information about you.

We use the limited information we collect to:

• Create and maintain your account.
• Provide and manage your subscription and access to the app.
• Respond to your support requests.
• Maintain the security and proper functioning of the app.
• Comply with our legal obligations.

We share information only with the service providers that make the app work, and only as needed for them to provide their service to us:

• Microsoft Entra — for secure sign-in and identity.
• Stripe — for payment processing and subscription billing.
• Microsoft Azure — for hosting our application and data.

These providers act as our processors and are bound by their own privacy and security obligations. We may also disclose information if required by law, or to protect the rights, safety, or property of NSPB Corporation, our users, or others.

We do not sell your personal information.

We keep your account and subscription information for as long as your account is active, and as needed to provide the app, comply with our legal obligations, resolve disputes, and enforce our agreements. When information is no longer needed, we take steps to delete it. (NSPB: confirm your specific retention periods with counsel and update this section if needed.)

You may:

• Access or update your profile information in the app.
• Cancel your subscription at any time.
• Request deletion of your account and associated personal information by emailing info@survey-scope.com. We will process your request as required by applicable law.

Depending on where you live, you may have additional rights regarding your personal information. Contact us at info@survey-scope.com to exercise any such rights.

We use industry-standard measures to protect your information, including authentication through Microsoft Entra and encrypted connections. No method of transmission or storage is completely secure, but we work to protect your information and to keep our systems up to date.

SurveyScope is a business tool intended for professionals and is not directed to children under 13 (or the equivalent minimum age in your jurisdiction). We do not knowingly collect personal information from children.

We may update this Privacy Policy from time to time. When we do, we will revise the "Effective date" above and post the updated policy at this location. Your continued use of the app after an update means you accept the revised policy.
